In consideration for allowing my child to participate in the Jump For Justice and by clicking on "I Agree”, I acknowledge, agree, warrant and covenant as follows:

I represent that I am the parent or guardian of a child who intends to participate in the Giveffect's Jump For Justice. I agree and acknowledge that my child is in good health and proper physical condition for the activities involved in and may participate in Jump For Justice and the on-line fund raising program. I further agree that if at any time I believe conditions or equipment to be unsafe or that my child should not or is incapable of continuing my child will immediately discontinue participation.

I also ACKNOWLEDGE that Giveffect's Jump For Justice may include risks that could result in SERIOUS INJURY. AND ACCEPT ALL RISKS KNOWN AND UNKNOWN AND ASSUME ALL RESPONSIBILITY FOR INJURIES, DAMAGES, LOSSES AND COSTS EVEN IF CAUSED IN WHOLE OR IN PART BY THE NEGLIGENGE of the Heart and Stroke Foundation , its affiliates, officers, directors, volunteers, employees, sponsors, advertisers and if applicable owners and lessor’s of premises on which the activities take place (each one a “Releasee”) and AGREE TO SAVE, HOLD HARMLESS, RELEASE AND INDEMNIFY the Releasees from all liability, claims, damages, demands, losses, and causes of action and any litigation expenses or legal costs arising out of my child’s participation in Jump For Justice and related activities whether it results from the negligence of any of the Releasees or from any other cause.
